The RN Home Services Case Manager utilizes clinical expertise and critical thinking skills to direct resource utilization, develop and implement a plan of care that is both cost‑effective and designed to assist patients in moving toward functional independence and improved quality of life. The RN Home Services Case Manager is the primary caregiver during admission, recertification, and discharge, assessing progress toward stated goals, completing the Outcome and Assessment Information Set (OASIS), and tracking progress against the admission assessment.
The incumbent coordinates caregivers and supplies necessary in the provision of patient care and works collaboratively with patients, families, healthcare providers, insurers, community resources, and other involved parties.
- Direct resource utilization to develop and implement a cost‑effective plan of care.
- Serve as the primary caregiver at admission, recertification, and discharge; assess progress toward stated goals.
- Complete OASIS and track progress against admission assessment.
- Coordinate caregivers and supplies necessary for patient care.
- Collaborate with patients, families, healthcare providers, insurers, community resources, and other stakeholders to ensure seamless communication and high‑quality outcomes.
- Lead coordination and delivery of exceptional patient care within the home setting as a central figure on the interdisciplinary team.
- Current Registered Nurse (RN) license in the state of practice
- Bachelor of Science in Nursing (BSN) from an accredited institution (degree verification required; must be obtained within four (4) years of hire or promotion)
- Basic Life Support Certification (BLS) for Healthcare Providers
- Valid driver’s license, insurance, reliable transportation, and an acceptable driving record
- Unit‑specific certification as required (e.g., ACLS, PALS)
- Demonstrated clinical nursing experience and knowledge of home health or hospice
- Effective verbal and written communication skills
- BSN from an accredited institution (degree verified)
- Case Management Certification
- One (1) year of experience working in home health or hospice
- Three (3) years of clinical nursing experience
- Experience working with third‑party payors
- Previous experience in case management, utilization review, and/or discharge planning
Ongoing need to see and read information, assess patient needs, operate monitors, and use complex equipment. Frequent verbal communication and rapid comprehension of alarms and emergency situations. Manual dexterity required for manipulating delicate equipment and frequent computer use for documentation. Ability to lift, carry, and transfer patients and supplies of varying weights, including heavy items. Ability to stand and walk throughout patient rooms, often navigating crowded areas with equipment and cords.
For driving roles, ability to operate a vehicle safely while seeing and reading traffic signs.

- Hourly rate: $41.20 – $62.17, based on experience
- Caregiver incentive up to $400 annually and a $1,500 referral bonus
