PS Nurse Practitioner - Heart Failure​/Transplant

Job Summary

The Division of Cardiovascular Medicine is looking for a motivated and enthusiastic individual to work on our Heart Failure/Transplant team. The ideal candidate is someone who has been practicing as a Nurse Practitioner for at least one year and who has experience with cardiac patients. • Assesses the physical condition of patients by performing examinations and obtaining medical histories.
• Formulates the medical plan and prognosis based on patient condition.
• Consults with physicians regarding patient health care plans and treatment.
• Orders, interprets, and evaluates diagnostic tests to identify and assess patients' clinical problems and health care needs.
• Performs selected invasive procedures for therapeutic and/or diagnostic reasons within scope of validated competence.
• Initiates and/or recommends medical drug therapies or other forms of related therapeutic treatment and procedures in accordance with approved protocols/policy.
• Complies with medical records policies regarding the complete and accurate documentation of patient care. Initiates and maintains required records and legal documents.
• Participates in quality improvement activities.
• Counsels patients, families and significant others about health and illness, and promotes health maintenance.
• Evaluates, plans, implements, and re‑evaluates the care of individuals requiring emergency measures.

Required

• Licensure to practice as a Nurse Practitioner in the State of Utah.

Preferred

• Certification in ACLS, BLS, NRP, or ATLS; may be required by department or privileging criteria.
• Utah State prescriptive privileges.
• Masters degree in Nursing or equivalency and National Certification for Nurse Practitioner.
• Experience in Cardiology (one year preferred).
• Immunization per CDC standards (except for documented medical or religious exemptions).

Pay Rate Range: $113,597 - $136,597 BOE
