Position Summary
Seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care, internal medicine, or geriatrics. The role involves providing personalized, coordinated healthcare for residents in long-term care settings, primarily nursing homes or assisted living communities enrolled in Medicare Advantage plans. Responsibilities include assessing patients' medical and mental health needs, developing care plans, prescribing medications, ordering diagnostics, and monitoring treatment responses.
The provider collaborates with a multidisciplinary team, exercises independent judgment, and ensures compliance with regulations. Qualifications include a valid APRN or PA license, a degree from an accredited program, and at least three years of clinical experience in geriatric populations. Proficiency in Electronic Health Records and Microsoft Office is required. Supervision duties may include staff training.
