PRN On-Call Hospice Nurse

The PRN On-Call Hospice Nurse is a part-time, on-site role based in the Salt Lake Valley area. This role is responsible for providing after-hours and weekend hospice nursing services, including in-person assessments, symptom management, phone triage and direct patient care in homes or facilities. Daily responsibilities include responding to on-call inquiries, triaging patient needs, coordinating with hospice teams, and implementing palliative care plans.

The nurse will provide education and emotional support to patients, families, and caregivers, documenting all care activities accurately and complying with hospice and regulatory standards. Collaboration with physicians, interdisciplinary team members, and community partners is essential to ensure high-quality, patient-centered end-of-life care.

• Strong clinical background in Nursing and Medicine, with the ability to deliver safe, evidence-based care.
• Experience in Palliative Care, Hospice and Home Health Care.
• Skill in Home Care and Direct Patient Care, including in-home assessments, interventions, and caregiver support.
• Active RN or LPN license in the state of Utah and current BLS/CPR certification.
• Prior hospice or end-of-life care experience preferred.
• Excellent communication, documentation, and interpersonal skills with patients, families, and interdisciplinary teams.
• Reliable transportation and willingness to travel locally for patient visits. Occasional travel to Heber, Ogden, or Tooele may be required.
• Must live with 45 minutes of downtown Salt Lake City.
• Required two shifts per month, and two holiday shifts (half-day) per year.

$4/hr for on-call time

$40 for routine hospice visit

$100 for admission

Weekdays:
Monday - Thurs. 5:00 PM - 9:00 AM
