Monday - Friday, Day shift with rotating on-call backup - Setting:
Field Services / Community Mental Health (Up to 500 sq mi area)
We are seeking an autonomous PMHNP to serve as the medical cornerstone of a multidisciplinary community mental health team. In this 36-hour/week role, you will provide psychiatric evaluations, medication management, and medical consultation for adults living with serious mental illness (schizophrenia, bipolar I, etc.) directly in the community rather than a traditional clinic.

- Clinical Care:
Conduct field-based psychiatric evaluations, risk assessments, and ongoing medication management utilizing shared decision-making. - Outreach & Monitoring:
Visit clients in homes and shelters; track lab work/metabolic health; coordinate medical referrals and hospital transitions. - Team
Collaboration:
Lead daily interdisciplinary team meetings, consult on complex psychiatric symptoms, and maintain compliant EMR documentation.
- Education:
MSN or DNP specializing in Psychiatric Mental Health Nursing. - Experience:
2+ years post-grad in community mental health or outpatient psychiatry serving serious mental illness (SPMI). MAT/buprenorphine experience preferred. - Licensure:
Utah APRN with prescriptive authority, ANCC PMHNP-BC certification, active DEA, CPR/BLS, and valid driver's license.
