Fourth Street Clinic (Wasatch Homeless Health Care), founded in 1988, has grown from a small triage clinic into a Federally Qualified Community Health Center with over 130 staff and a large volunteer network. The clinic serves nearly 6,000 individuals and families experiencing homelessness each year, providing close to 30,000 medical, mental health, substance use disorder, dental, and case management visits. Its on-site ALSAM Foundation Pharmacy dispenses more than 100,000 medications annually, supporting comprehensive, continuous care.

By increasing access to integrated primary health care for unhoused Utah residents, Fourth Street Clinic plays a critical role in ending homelessness, promoting community health, and reducing overall health care costs.

The Registered Nurse is a full-time, on-site role located in Salt Lake City, UT, providing direct, trauma-informed nursing care to patients experiencing homelessness. Day-to-day responsibilities include performing patient assessments, triage, and health screenings, administering medications and treatments, and supporting chronic disease and preventive care management. The RN collaborates with medical, behavioral health, dental, pharmacy, and case management teams to coordinate patient care, support care plans, and facilitate referrals.

The role also involves patient education on health conditions, self-care, and medication adherence, documenting care accurately in the electronic health record, and contributing to quality improvement and infection control practices. The RN maintains a safe, inclusive environment for patients and staff while responding to urgent and emergent situations according to clinical protocols.

• Active Registered Nurse (RN) license in the state of Utah; current BLS certification and eligibility to work in a community health or primary care setting.
• Clinical skills in patient assessment, triage, medication administration, wound care, and chronic disease management.
• Experience or strong interest in working with individuals experiencing homelessness, substance use disorders, and mental health conditions in a trauma-informed, culturally responsive manner.
• Proficiency with electronic health records, documentation standards, and basic computer skills (email, scheduling, data entry).
• Strong communication and interpersonal skills, including ability to build trust, maintain professional boundaries, and work effectively in an interdisciplinary team.
• Ability to manage multiple priorities in a fast-paced environment, sound clinical judgment, and commitment to patient safety and quality improvement.
• Flexibility to adapt to changing patient needs and clinic workflows; reliability in attendance and punctuality for an on-site, full-time schedule.
• Associate or Bachelor’s degree in Nursing; experience in community health, public health, emergency, or primary care nursing is preferred.
