Job Summary

The Permanent Supportive Housing Case Manager provides trauma-informed, family-driven case management services to unhoused families as they transition from homelessness into stable housing throughout Salt Lake County. This role works closely with each family to assess their unique goals, identify needs, and address barriers to self-sufficiency. The Case Manager connects families to vital community resources and partners with them to develop personalized plans that support long-term housing stability and overall well-being.

This position requires a deep commitment to serving families with compassion, respect, and a strengths-based approach that empowers them to thrive in their new homes.

Essential Duties and Responsibilities

  • Provide trauma-informed case management services to families, including outreach, ongoing assessments, goal setting, and creative problem-solving using a network of community resources.
  • Meet with families in their homes, at emergency shelters, and in other community locations to assess needs and collaboratively establish goals.
  • Assist families in identifying barriers to stability and connecting them with appropriate community resources and support services.
  • Demonstrate a strong belief in the Housing Focused model and engage families in ongoing, supportive conversations about housing.
  • Help families obtain income through employment, benefits, and other resources to support long-term stability.
  • Assess children's needs and make appropriate community referrals, including liaising with schools to coordinate supportive services.
  • Focus on housing and strength-based practices to meet the unique needs of each family.
  • Collaborate daily with all departments to promote a supportive and effective team environment.
  • Build and maintain relationships with community partners to identify housing options and additional resources for families.
