We're looking for a compassionate and organized Residential Treatment Case Manager to help clients successfully transition from treatment to long-term stability. In this role, you'll work closely with individuals who are experiencing homelessness or are at risk of becoming homeless, helping them identify goals, overcome barriers, and connect with the resources they need to thrive.
From securing housing and employment to coordinating healthcare and community services, you'll play a vital role in supporting each client's recovery journey while collaborating with a dedicated multidisciplinary team.

Salary: $47,000 annually

What You'll Do
- Screen clients for program eligibility
- Complete comprehensive needs assessments
- Develop individualized case management plans and monitor client progress
- Meet regularly with clients to support their goals and recovery
- Connect clients with housing, employment, education, healthcare, and other community resources
- Collaborate with counselors, case managers, and community partners to coordinate care
- Prepare progress reports and outcomes for grant requirements and other stakeholders
- Maintain accurate case notes, client information, and billing documentation in KIPU
- Use internal tracking systems to ensure timely and accurate billing
- Provide administrative support as needed and assist with other duties that support the program's success
What We're Looking For
- Case management experience is required
- Active Utah Social Service Worker (SSW) license preferred
- Bachelor's degree in Social Work or a related field preferred
- Experience working in substance use treatment or behavioral health preferred
- Strong communication, organization, and relationship-building skills
- A passion for helping individuals achieve stability, independence, and lasting recovery
