44 North Case Manager

We are seeking a compassionate, motivated, and recovery-oriented Case Manager to join our 44 North team. This role provides an opportunity to support individuals experiencing serious mental illness and co-occurring conditions as they work toward housing stability, recovery, and community integration.

As a 44 North Case Manager, you will play a vital role in helping residents maintain stable housing, access community resources, develop life skills, and achieve their self-identified goals. You'll work alongside a multidisciplinary
