Position: Supportive Housing Program Assistant

Permanent Supportive Housing Program Assistant/Case Manager

Salt Lake City, UT | Full-Time | 36-Hour Work Week

$52,000–$62,000 annually

At First Step House, "We help people build lives of meaning, purpose, and recovery." We are looking for an organized, compassionate, and adaptable professional to join our Permanent Supportive Housing team.

This unique role combines case management and program administration. You'll work directly with residents who have experienced homelessness and may be living with mental illness and/or substance use disorders, while also supporting the organization and day-to-day operations of the program. You'll work alongside clinical, housing, medical, and other support staff to help residents maintain housing, access benefits and community resources, and build greater stability and independence.

What You’ll Do
  • Develop service plans with residents and help them work toward their goals.
  • Support residents in maintaining housing, benefits, healthcare, and overall stability.
  • Connect residents with community resources and coordinate services with outside providers.
  • Provide trauma-informed, person-centered support using motivational interviewing and de-escalation strategies.
  • Collaborate with clinical, medical, peer support, and other community partners.
  • Complete timely and accurate documentation in electronic health record and HMIS systems.
  • Manage referrals, waitlists, intakes, and discharge processes.
  • Conduct intake appointments and complete admissions paperwork.
  • Maintain spreadsheets, reports, files, and program records.
  • Coordinate appointments, meetings, and care coordination activities.
  • Communicate with residents, community partners, providers, and prospective residents.
  • Assist with general administrative and program needs.
Qualifications
  • A Bachelor's degree in Social Work, English, business, or other related field is preferred
  • Case management experience highly preferred
  • Experience with mental health and substance use treatment are preferred
  • Valid Drivers License

  • Knowledgeable about or experienced working with homelessness, mental illness, substance use, or other barriers to stability.
  • Committed to maintaining professional boundaries, confidentiality, and ethical standards.
  • Able to work collaboratively as part of a multidisciplinary team.
  • Someone who demonstrates an Outward Mindset and treats residents and coworkers with dignity and respect.
