Key Responsibilities- Provide case management to people who transition from Intermediate Care Facilities into
Home and Community-Based Services. - Provide education to individuals and families regarding Home and Community-Based Services and available service options.
- Provide information concerning rights, options, benefits, services, Person Centered Planning goals and expectations.
- Complete field work assignments used to implement Person Centered Support Plans(PCSP), budgets, RFS tasks, Charting the Life Course, and UCANSassessments.
- Coordinate and refer services of community resources, placement providers, collateral agencies, the courts, and/or other community partners.
- Collaborate with other state and local programs, organizations and agencies, and with various stakeholders to promote program goals and objectives.
- Provide consultation, make recommendations, give appropriate advice, and/or assist to facilitate decisions.
Minimum or "day-one" qualifications:
- Hasa Bachelor's Degree with at least 1 year of relevant experience working directly with people with disabilities.
- Has knowledge of Home and Community Based Services (HCBS) and Settings Rule.
- Has obtained certification as a QIDP (Qualified Intellectual Disability Professional).
Preferred or trainable/on-the-job qualifications:
- Hasstrong interpersonal skills.
- Is passionate about empowering people with disabilities with working knowledge ofHome and Community-Based Services (HCBS), the Community Transition Waiver andHCBS Settings Rule
- Isan excellent listener who is also very proactive
- Has knowledge of community resources for people with disabilities and how to access those resources
- Hasthe ability to work well with a wide range of people with varying interests
- Can prioritize multiple tasks while maintaining attention to detail
- Is self-motivated with ability to work a flexible schedule
