• Develops and revises plan of care in consultation with physician and other care team members.
• Provides therapeutic treatments and evaluation of equipment needs as applicable.
• Participates in clinical conferences and in-service programs.
• Conducts onsite supervisory visits of physical therapy assistants.
• Advises and consults with the family and other agency personnel as applicable.
• Prepares clinical and progress notes.
• Protects and honors patient and coworker confidentiality and the right to privacy.
• Evaluates outcomes of care.
• Plans for discharge.

• Current Texas PT License.
• Current Certification in BSL/CPR.
• Valid Texas Driver’s License and Automobile Insurance.
• Knowledge of Well Sky EMR software is a plus.
