Job Summary
The social worker provides complex social services to patients and their families from admission to discharge, collaborating with the interdisciplinary team to ensure comprehensive care.
Key Responsibilities- Assess patient needs and develop appropriate intervention plans.
- Coordinate services, support discharge planning, and facilitate communication among care providers.
- Maintain accurate documentation and ensure compliance with regulations.
- High School Diploma, GED, or equivalent.
- Degree in social work (level dependent on licensure requirements).
- Preferred: 1 year as Healthcare/Medical Social Worker, 1 year as Healthcare/Medical Case Manager, and 2 years of counseling experience.
- Licensed Master Social Worker (LMSW) or Licensed Baccalaureate Social Worker (LBSW) with minimum two years of social work experience (current in Texas).
- Basic Life Support (BLS) certification within 90 days of start.
- SAFE Level III Training Course completion within six months.
