LPN​/LVN

Join Encompass Health as an LPN/LVN in our inpatient rehabilitation hospital. Provide direct patient care, administer medications, monitor vital signs, and support individualized rehab plans under RN and physician direction. Collaborate with an interdisciplinary team to promote safe recovery, patient education, and discharge readiness. Responsibilities

• Provide direct bedside nursing care to rehab patients under RN/physician supervision
• Administer medications and treatments per orders and facility policies
• Monitor, document, and report patient status, vital signs, and responses to treatment
• Assist with ADLs, mobility, and rehabilitative activities to support recovery goals
• Participate in care planning and interdisciplinary team conferences
• Educate patients and families on care plans, medications, and safety
• Maintain accurate and timely electronic medical records and documentation
• Ensure infection control, safety, and regulatory compliance at all times
• Coordinate with therapists and support staff to optimize patient outcomes
• Support orientation of new staff and contribute to a positive team culture
