LVN, LPN​/LVN, Healthcare Nursing

Encompass Health is seeking a Licensed Vocational Nurse (LVN) to join our Healthcare & Medical Services team in our inpatient rehabilitation setting. The LVN will deliver high-quality, patient-centered nursing care, including assessments, medication administration, wound care, and coordination of interdisciplinary rehab plans. You'll collaborate closely with RNs, therapists, and physicians to support functional recovery and safe discharge planning. Responsibilities

• Provide direct bedside nursing care to rehab patients under RN and physician direction
• Administer medications and treatments per provider orders and facility protocols
• Perform patient assessments and monitor vital signs and responses to therapy
• Support development and implementation of individualized rehabilitation care plans
• Assist with ADLs, mobility, transfers, and safety precautions to prevent falls
• Document care, assessments, and patient progress accurately in the medical record
• Communicate effectively with patients, families, and interdisciplinary team members
• Reinforce patient and family education on disease management and rehab goals
• Follow infection control, patient safety, and quality standards at all times
• Participate in shift reports, team conferences, and ongoing performance improvement
