Bloom Healthcare has been voted the "Top Workplace" for six consecutive years. We are expanding into the San Antonio market and are seeking dedicated primary care providers to bring our model of care to residents of San Antonio and surrounding communities.

• Deliver comprehensive primary care at home and in senior communities for patients.
• Represent Bloom Healthcare in the San Antonio market as an ambassador, supporting market growth, including market analysis of the local landscape and competitors.
• Strategic planning to grow your patient panel and provide primary care at home visits to 10 patients per day.
• Build trusted relationships with care team members across assisted living facilities, hospitals, clinics, and community organizations to coordinate care.
• Collaborate with interdisciplinary teams, including specialists, pharmacists, and social workers, to coordinate care and achieve optimal health outcomes.
• Provide data-driven, condition-specific specialty visits as needed.
• Manage health outcomes of an empaneled patient population.
• Engage with population health data to drive superior patient outcomes.
• Attend ongoing educational opportunities.
• Commute to assigned assisted living communities to provide care at home.
• Occasional travel to the Denver market for initial and ongoing training.

• Scheduled Days:
  Monday - Friday (3, 4, or 5 days)
• Relocation assistance packages available
• Flexible schedules with rotating on-call (2-3x per year)

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Texas typically falls between $225,000 - $260,000 annually with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on qualifications, experience, and location within the state.
