PRN Hospice Social Worker (MSW)

Varco Hospice is seeking a compassionate and hospice-experienced Social Worker (MSW) to join our interdisciplinary team. This is a PRN position only supporting patients and families throughout the end‑of‑life journey.

• Provide medical social services in accordance with the patient’s Plan of Care
• Complete psychosocial assessments within 5 days of hospice admission
• Support patients and families with end‑of‑life planning, coping, and advocacy
• Assist with community resources, advance planning, and discharge planning as needed
• Observe, document, and report changes in patient and family psychosocial status
• Participate in IDG and QAPI meetings
• Maintain accurate, timely clinical documentation
• Serve as a resource and consultant to the interdisciplinary team

• Active Texas Social Work license (MSW required; BSW considered per hospice regulations)
• Minimum of 2 years hospice social work experience required
• One year of healthcare social work experience
• Current TB or chest X‑ray and CPR certification
• Valid driver’s license and willingness to travel to patient homes and facilities
• Strong communication and documentation skills
