Overview

FULL TIME - OCCUPATIONAL THERAPIST - PARKLANE WEST REHABILITATION - SAN ANTONIO, TX

Parklane West Rehabilitation in San Antonio, TX is a post-acute care facility offering long-term care, short-term care, and geriatric services. We provide a stable and friendly in-house therapy team and excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a full-time Occupational Therapist for our in-house rehab program.

Responsibilities
  • Effectively screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following regulatory and clinical practice standards.
  • Utilize standardized assessments and evidence-based practice to support clinical interventions.
  • Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administer various procedures as part of the rehabilitation plan including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.
  • Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluate treatment outcomes for effectiveness. Modify services based on patient evaluation; make referrals as appropriate based on outcomes.
  • Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
  • Communicate effectively with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provide a comprehensive discharge summary of services provided per regulatory and clinical requirements.
Qualifications

Occupational Therapist license is required. Open to Occupational Therapists at all experience levels; new grads are welcome.
