We are seeking an experienced Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility (SNF) setting. The Physical Therapist will play a key role in evaluating residents, developing individualized treatment plans, and delivering high-quality rehabilitative care to improve mobility, function, and overall quality of life.

• Evaluate patients’ physical conditions and functional limitations
• Develop and implement individualized treatment plans
• Provide therapeutic exercises, mobility training, and pain management interventions
• Monitor patient progress and modify treatment plans as needed
  
  Maintain accurate and timely clinical documentation
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care
• Educate patients and caregivers on treatment plans and home exercise programs
• Comply with all facility policies, procedures, and regulatory standards

• Active State Physical Therapist license (must be current)
• Minimum of 1 year of experience as a Physical Therapist
• Experience in a Skilled Nursing Facility or rehabilitation setting preferred
• Strong clinical, communication, and documentation skills
• Ability to work independently and as part of a multidisciplinary team
