POSITION SUMMARY/RESPONSIBILITIES
This position will serve as the link between the community, volunteer network members, health system providers, social workers, and the Community Health Division in order to increase the utilization of available health promotion programs and maximize the use of existing health services as deemed appropriate. Assists and guides patients through community health prevention and treatment services to ensure that patients are able to access and keep medical appointments, participate in recommended care plans, and receive appropriate health information about their diagnosis and treatment plan.
Assists and supports the marketing efforts of the Health Promotion Department in community venues including, but not limited to, health fairs, University Health sponsored events, educational classes provided within the Women's Health/Newborn Services service lines, and educational classes on chronic illness provided through the Texas Diabetes Institute. All functions and behaviors are applicable to the clinical and community settings.
EDUCATION/EXPERIENCE
Bachelor's degree in social sciences, health education or related field of study is required. Four years of experience in the area of health education, public health, social work and/or case management may be considered as a substitute in lieu of the Bachelor's degree. English/Spanish fluency is required.
