Job Summary
Under general direction, is responsible for coordinating, implementing, and monitoring development plans and support services for unsheltered homeless clients to help them obtain housing and achieve self‑sufficiency through street outreach. Provides case management and monitors client participation and progress, assisting them in overcoming barriers and enhancing their employment and housing potential and opportunities. Exercises direct supervision.

7:45 a.m. – 4:30 p.m., Monday – Friday with occasional evenings, weekends and holidays.
Essential Job Functions- Travels to encampment sites and locations throughout San Antonio where the unsheltered population resides and proactively initiates non‑judgmental conversations to create and maintain relationships and provide a consistent presence.
- Provides clinical coordination, case management, and crisis intervention services to assist clients with service navigation, employment, and stabilizing their mental health and medical situations, including mental illness, substance use disorders, or other medical concerns.
- Conducts assessments and diagnosis of behavioral health disorders, develops behavioral health treatment plans, and provides counseling for families, couples, and individuals enrolled in the homeless outreach program.
- Coordinates with community organizations, neighborhood associations, government organizations, faith‑based organizations, council district offices, Homeless Hotline, 311, law enforcement, the SAPD San Antonio Fear Free Environment Unit, and other service providers, which involves attending meetings and events outside normal work hours.
- Develops strong community partnerships that enhance the homeless outreach program's effectiveness.
- Analyzes complex data, policies, practices, systems, and procedures; develops recommendations; prepares reports; and delivers presentations.
- Collects and inputs data into databases.
- Performs related duties and fulfills responsibilities as required.
- Master's Degree in Social Work, Psychology, Counseling, or other social services programs from an accredited college or university.
- Three (3) years of experience in social work or related experience.
- Valid Class "C" Driver's License.
- Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) from the Texas Department of State Health Services.
- Licensed Clinical Social Worker – Supervisor (LCSW‑S) certification.
- One (1) year of supervisory experience.
- Experience with homeless outreach and/or homeless services.
- Experience with unsheltered population.
- Experience with Harm Reduction techniques.
- Knowledge of Trauma Informed Care and Motivational interviewing techniques.
- Willing to work outside of normal work hours during periods of inclement weather and/or city closure.
- Applicants may substitute two years of related full‑time experience for one year of higher education or one year of related higher education for two years of experience to meet the minimum requirements.
- Applicants selected for employment must receive satisfactory results from pre‑employment drug testing and background checks. Additional checks, including a physical and motor vehicle record evaluation, may be required.
- Required employment history information will be needed for verification. Military service may be verified with a DD214.
- Official transcripts, diplomas, certifications, and licenses must be submitted at the time of processing.
- Knowledge of counseling, psychology, ethics, and social services theory and methodology.
- Knowledge of standard program evaluation methods and reporting procedures.
- Knowledge of Housing First and Harm Reduction practices.
