Position: Weekend Nurse Practitioner (Skilled Nursing) Avir at San Antonio

Nurse Practitioner (Skilled Nursing) Major Responsibilities

  • Act as the Skilled Nursing Facility’s leading provider of internal medicine services on Saturday and Sunday.
  • Collaborate with the interdisciplinary team and physicians to reduce hospitalizations on weekends.
  • Work with the entire patient population—short and long-term residents—with assistance of nursing staff and guidance from medical directors and attending physicians.
  • Evaluate new-admission visits.
  • Coordinate discharge visits.
  • Evaluate patients exhibiting changes in condition.
  • Provide types of care, including advanced care planning, comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol for acute and chronic illness and disease.
  • Prescribe all necessary medications and treatments.
  • Perform comprehensive physical examinations.
  • Develop and implement patient management policies and procedures.
  • Facilitate referrals to other healthcare professionals and medical facilities.
  • Communicate with attending physicians and/or specialists regarding patient care.

Qualifications
  • Advanced Practice Licensure (APRN) with a Texas license.
  • Active license and DEA number.
  • Previous experience in nursing or other medical fields.
  • Ability to build rapport with patients and their loved ones.
  • Ability to thrive in a fast-paced environment.
  • Excellent written and verbal communication skills.
  • Strong leadership experience with emphasis on teamwork in a healthcare setting.
  • Master’s degree required.
