Patient Care Manager – RN (Director of Nursing), Hospice Care
IntroductionOur client, a leading provider in the healthcare industry, is seeking an experienced and compassionate Patient Care Manager – RN (Director of Nursing) to lead their hospice team. This is a rewarding opportunity to guide and inspire a team committed to delivering exceptional end-of-life care, combining clinical excellence with deep empathy for patients and families.
Responsibilities- Lead, mentor, and inspire a multidisciplinary team of hospice nurses and clinical staff.
- Oversee day-to-day clinical operations to maintain the highest standards of patient care and regulatory compliance.
- Foster a mission-driven and supportive team culture, encouraging professional growth and collaboration.
- Partner with interdisciplinary teams—including physicians, social workers, and therapists—to provide holistic care tailored to each patient’s needs.
- Monitor and analyze quality metrics, implementing continuous improvement initiatives to enhance outcomes.
- Ensure compliance with all applicable federal, state, and local hospice regulations, as well as organizational policies and best practices.
- Support and advance staff development, providing ongoing education and promoting clinical excellence.
- Participate in strategic planning and contribute to the organization’s growth and innovation objectives.
- Serve as a clinical resource, offering guidance and stepping in as needed to exemplify outstanding leadership.
- Current Registered Nurse (RN) license in the state of practice.
- RN required; BSN preferred
- Minimum of 2 years’ experience in hospice or palliative care.
- At least 2 years of leadership or management experience in a clinical setting.
- Strong knowledge of hospice regulations, compliance standards, and best practices.
- Exceptional leadership, communication, and organizational skills.
- Ability to mentor and motivate teams, fostering a collaborative and supportive environment.
- Demonstrated ability to approach challenges with innovation and compassion.
