Patient Care Manager – RN (Director of Nursing), Hospice Care

Introduction

Our client, a leading provider in the healthcare industry, is seeking an experienced and compassionate Patient Care Manager – RN (Director of Nursing) to lead their hospice team. Responsibilities
  • Lead, mentor, and inspire a multidisciplinary team of hospice nurses and clinical staff.
  • Oversee day-to-day clinical operations to maintain the highest standards of patient care and regulatory compliance.
  • Foster a mission-driven and supportive team culture, encouraging professional growth and collaboration.
  • Partner with interdisciplinary teams—including physicians, social workers, and therapists—to provide holistic care tailored to each patient’s needs.
  • Monitor and analyze quality metrics, implementing continuous improvement initiatives to enhance outcomes.
  • Ensure compliance with all applicable federal, state, and local hospice regulations, as well as organizational policies and best practices.
  • Support and advance staff development, providing ongoing education and promoting clinical excellence.
  • Participate in strategic planning and contribute to the organization’s growth and innovation objectives.
  • Serve as a clinical resource, offering guidance and stepping in as needed to exemplify outstanding leadership.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • RN required; BSN preferred
  • Minimum of 2 years’ experience in hospice or palliative care.
  • At least 2 years of leadership or management experience in a clinical setting.
  • Strong knowledge of hospice regulations, compliance standards, and best practices.
  • Exceptional leadership, communication, and organizational skills.
  • Ability to mentor and motivate teams, fostering a collaborative and supportive environment.
  • Demonstrated ability to approach challenges with innovation and compassion.
