Position Summary
The Nurse Practitioner will work collaboratively with a dedicated clinical care team to provide evidence-based care to a panel of patients. The provider will be responsible for assessment, diagnosis, treatment, management of health conditions, patient education, health counseling, and disposition planning. This role reports to the Practice Manager and focuses on delivering high-quality patient outcomes through coordinated care.
Required Qualifications- Master's Degree from an accredited Family Nurse Practitioner program.
- Current National Board Certification as a Family Nurse Practitioner.
- Current state Nurse Practitioner license.
- Current RN and APRN licenses in good standing with no disciplinary actions.
- Current BLS certification from the American Heart Association (AHA) or American Red Cross (ARC).
- Minimum of 1 year of recent Nurse Practitioner experience in:
- Primary Care
- Family Medicine
- Urgent Care
Candidates without prior Primary Care or Urgent Care experience will not be considered.
Key Responsibilities- Provide evidence-based Primary and Family Care services.
- Perform comprehensive patient assessments, diagnosis, treatment, and follow-up care.
- Develop and manage treatment plans for acute and chronic conditions.
- Conduct routine examinations and preventive health screenings.
- Promote healthy lifestyle choices and patient wellness initiatives.
- Coordinate care with physicians, specialists, and other healthcare professionals.
- Support chronic disease management programs.
- Educate patients regarding treatment plans, medications, and preventive care.
- Complete and maintain accurate electronic medical records.
- Perform clinic administrative functions including:
- Opening and closing procedures
- Inventory management
- Insurance verification
- Payment collection
- Patient follow-up
- Maintaining a clean and safe clinical environment
- Strong understanding of quality measures and chronic disease management.
- Excellent verbal, written, and electronic communication skills.
- Strong organizational and multitasking abilities.
- Self-motivated with strong problem-solving skills.
- Ability to work independently and autonomously.
- Adaptability and flexibility in a fast-paced clinical environment.
- Proficiency with healthcare technology and information systems.
- Willingness to obtain additional state licenses if required.
- DEA registration may be required during onboarding.
- Position may require working every other weekend.
- Holiday rotation coverage may be required.
- This is an onsite, patient-facing role.
- Ability to work collaboratively within a clinical care team environment.
