Pay Rate: $74.87 per hour
OverviewNurse Practitioners in this role work in collaboration with a dedicated clinical care team to deliver evidence-based care to a defined panel of patients. The care team supports daily operations to improve outcomes across the patient population. Responsibilities include comprehensive assessment, diagnosis, treatment, and management of acute and chronic health conditions, patient education and counseling, and disposition planning. The position reports directly to the practice manager.
This is a weekend-focused schedule with a “Weekend Workforce” model. Providers are expected to work at least 5 out of every 6 weekends, including both Saturday and Sunday. The role is primarily based in a home clinic setting, with potential float coverage opportunities depending on operational needs and interest in additional weekday hours.
Schedule- Primarily weekends (Saturday and Sunday required)
- Minimum 5 of 6 weekends scheduled
- Optional additional weekday hours available depending on staffing needs and candidate preference
- Home clinic assignment with possible float support across nearby sites
- Provide direct patient care through assessment, diagnosis, treatment, and management of conditions
- Develop and implement patient care plans in coordination with the clinical care team
- Conduct patient counseling, education, and health promotion activities
- Order and interpret diagnostic tests as appropriate
- Ensure accurate documentation of all clinical encounters
- Support continuity of care and panel management objectives
- Collaborate daily with physicians, nurses, and support staff to improve outcomes
- Active Nurse Practitioner license in the state of Texas
- Experience in primary care or urgent care required
- Strong clinical assessment and diagnostic skills
- Ability to work independently within a collaborative team structure
- Comfortable with weekend-only or weekend-dominant scheduling model
- Strong documentation and EMR proficiency
- Prior work in outpatient primary care settings
- Urgent care or high-volume clinic experience
- Experience managing patient panels and chronic disease care
