Job Description

The LVN will provide direct nursing services for adults with severe and persistent mental illness, assist with medical treatments, complete documentation for clients, provide nursing services including medication injections, assist physicians and team with caseloads, educate clients on medications and general health issues. Along with psychiatrist and Nurse Practitioner, the nurse serves as the medical specialist on the ACT team. Assist with medical treatments/interventions appropriately, taking initial health history on new patients and conducting a monthly or quarterly assessment for all clients on their assigned team.

Transport consumers to sites of medical interest (as needed) – this position is field‑based nursing, employee will provide visits to consumers in their homes and the community. Other duties as assigned.
