In-Home - LVN

We are looking for an In‑Home Care Licensed Vocational Nurse (LVN) to work full‑time, part‑time or PRN. This role offers flexible schedule options, a warm and supportive environment, and an opportunity to make a real difference in patients’ lives.

• Travel to assigned patient’s homes and manage their care plan according to the physician’s instructions.
• Assess and chart observations of the patient’s condition at each visit.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Dress or redress wounds and assess the healing progress.
• Educate caregivers and family on aftercare or ongoing care of the patient.
• Keep the patient comfortable through palliative care.
• Recommend tools or devices that may improve the quality of life for the patient.

• Strong interpersonal skills that allow building personal rapport and establishing trust with patients.
• Initiative to make independent decisions when required while accepting direction and requests from patients.
• Ability to follow instructions but also act independently if necessary.
• Excellent observational and problem‑solving skills.
• Ability to act swiftly in emergencies and work efficiently.
• Flexibility and patience to work in a variety of living environments.
• Knowledge of clinical skills including changing bandages, giving tube feedings, administering shots, and setting up and monitoring IVs.

• Relevant license and certification.
• A valid driver’s license and reliable transportation.
• Strong candidates have a minimum of one year of experience working in the home‑care industry (or related industry – hospice, home health, hospital, etc.).
