Role Description
This is a full-time, on-site Licensed Vocational Nurse (LVN) role based in the San Antonio, Texas Metropolitan Area. The LVN will provide direct patient care in the home setting, including monitoring vital signs, administering medications and treatments as ordered, performing wound care, and supporting activities of daily living within the LVN scope of practice. The role involves following individualized care plans, documenting assessments and interventions accurately, and communicating changes in patient condition to the supervising registered nurse and healthcare team.
The LVN will educate patients and caregivers on disease management, medications, and home care safety, while promoting comfort, dignity, and independence. Day-to-day work includes coordinating visit schedules, adhering to infection control and safety standards, and collaborating with interdisciplinary team members to support high-quality, compliant home health services.
- Current, active Licensed Vocational Nurse (LVN) license in the state of Texas, in good standing.
- Proven clinical skills in basic nursing care, medication administration, vital sign monitoring, and wound care within LVN scope of practice.
- Strong patient and family communication skills, including the ability to explain care instructions and provide health education in a clear, compassionate manner.
- Ability to accurately document care, follow care plans, and use electronic health record (EHR) systems or related documentation tools.
- Demonstrated reliability in time management, visit scheduling, and prioritizing multiple patients and tasks in a home health environment.
- Knowledge of infection control, safety protocols, and regulatory guidelines relevant to home health and vocational nursing practice.
- Capacity to work collaboratively with registered nurses, therapists, physicians, and other team members while also functioning independently in the field.
- Valid driver’s license, reliable transportation, and willingness to travel within the San Antonio, Texas Metropolitan Area for patient visits.
- Prior home health, hospice, or community-based care experience preferred but not required; new graduates with strong clinical skills and a commitment to learning are encouraged to apply.
- CPR/BLS certification and ability to meet all pre-employment health, background, and immunization requirements.
