What You'll Be Doing:

• Conducting patient assessments (start-of-care, post-hospital, and reassessments) with a focus on personalized, patient-centered care.
• Creating, implementing, and evaluating care plans alongside physicians, caregivers, and the patient.
• Using OASIS data for ongoing assessments, ensuring optimal care at every visit.
• Providing expert wound care and other nursing treatments to patients.
• Maintaining accurate, timely documentation via CradleMRx software.
• Complying with all health and regulatory standards.

Required Qualifications:

• Graduate of an accredited nursing school.
• Current Texas RN license (or compact license).
• IV certification (required) and BLS certification (required).
• 1+ years of experience in home health care (or similar environment).
• Wound care expertise is a must.
• Ability to assist with patient mobility and transfers.
• Access to reliable transportation and a valid driver's license.
• Proof of insurance for your vehicle.
