Job Summary:

As an Evening Shift Licensed Vocational Nurse (LVN) at Avir at Heritage, you will play a vital role in providing exceptional care to our residents in San Antonio, Texas. This opportunity supports a dynamic team of healthcare professionals who are committed to delivering high-quality patient care.

• Provide direct patient care to residents with diverse and complex medical needs under the supervision of a registered nurse
• Administer medications, treatments, and therapies as prescribed by physicians
• Monitor patients’ vital signs, report changes, and document observations accurately
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Perform basic wound care, monitor vital signs, and assist with activities of daily living (ADLs)
• Maintain accurate and detailed records of patient care and treatment
• Provide emotional support and comfort to residents, families, and visitors
• Participate in quality improvement initiatives and maintain a clean and safe environment
