LVN Clinical Coordinator – Boerne, TX
Oversees daily clinical operations, manages patient schedules, and ensures care aligns with regulatory standards. Acts as a liaison between nurses and therapists, handling physician orders and conducting chart audits. Interacts with administrative and clinical staff, including front desk, medical records, nursing, and ancillary services when applicable, to maximize resource utilization and ensure efficient delivery of services.
Coordination & ComplianceEnsures clinical services are delivered per the patient’s care plan and meet state and federal regulations. Reviews clinical notes.
CommunicationManages referrals, takes verbal or written orders from physicians, and updates At Risk Registries.
Software ProficiencyUtilizes industry‑standard EMR systems such as Kinnser/Well Sky.
QualificationsEducation: LVN Certified
Experience: Minimum 1–2 years of home‑health clinical experience, including wound care.
Skills & Abilities- Strong leadership and effective communication skills.
- Excellent planning, organization, and management abilities.
- Ability to multi‑task and demonstrate teamwork cooperation.
- Manages medical supplies orders, distribution, and inventory.
Full‑time, Monday through Friday with a rotating on‑call schedule assigned. May involve travel between office and supply storage.

PayBase Salary: $65,000–$70,000.
