South Texas Community Living is hiring a full-time LVN to provide nursing coordination to individuals with intellectual disabilities in group home and natural home settings. This position is similar to home-health nursing, with some duties and documentation specific to meet the unique requirements of HCS and/or ICF regulations. Our program provides a rewarding job experience for anyone interested in promoting the independence of people with disabilities.
ShiftSchedule
FT Shift(s):
Monday – Friday
Hours:
11:00 AM – 7:00 PM or 7:00 AM – 3:00 PM
- Patient assessment
- Monitoring unlicensed personnel (direct care staff) in the performance tasks delegated by the supervising RN.
- Training of direct care staff
- Ordering medications and monitoring medication supplies
- Coordinating medical appointments.
- Communicating medical issues to family members of patients
- General nursing tasks as assigned.
- Health insurance (after 90 days)
- 401(k) (after 1 year)
- Paid time off (immediate accrual)
- Valid LVN license required
- Valid and current Texas driver's license
- Clear background check
- Clear DMV check
- High school diploma or equivalent proof of education
- No confirmation of abuse/neglect/exploitation of an elderly or disabled person
- No history of Medicaid fraud.
- Experience working with people with intellectual and developmental disabilities (group home experience preferred)
- Strong communication and team-participation skills; friendly, positive attitude.
- General computer skills (not necessarily expert); be able to use Word, Excel, Outlook, and general internet navigation.
- Must have excellent time-management skills.
- Must be capable of multi-tasking in a fast-paced environment.
