South Texas Community Living is hiring a full-time LVN to provide nursing coordination to individuals with intellectual disabilities in group home and natural home settings. This position is similar to home-health nursing, with some duties and documentation specific to meet the unique requirements of HCS and/or ICF regulations. Our program provides a rewarding job experience for anyone interested in promoting the independence of people with disabilities.

Shift

Schedule

FT Shift(s):
Monday – Friday

Hours:

11:00 AM – 7:00 PM or 7:00 AM – 3:00 PM

LVN Duties
  • Patient assessment
  • Monitoring unlicensed personnel (direct care staff) in the performance tasks delegated by the supervising RN.
  • Training of direct care staff
  • Ordering medications and monitoring medication supplies
  • Coordinating medical appointments.
  • Communicating medical issues to family members of patients
  • General nursing tasks as assigned.

Qualifications
  • Valid LVN license required
  • Valid and current Texas driver's license
  • Clear background check
  • Clear DMV check
  • High school diploma or equivalent proof of education
  • No confirmation of abuse/neglect/exploitation of an elderly or disabled person
  • No history of Medicaid fraud.
  • Experience working with people with intellectual and developmental disabilities (group home experience preferred)
  • Strong communication and team-participation skills; friendly, positive attitude.
  • General computer skills (not necessarily expert); be able to use Word, Excel, Outlook, and general internet navigation.
  • Must have excellent time-management skills.
  • Must be capable of multi-tasking in a fast-paced environment.
