Rehabilitation Institute of South San Antonio, a 36‑bed inpatient acute rehabilitation hospital located at 12011 SE Loop 410 Access Road, San Antonio, Texas. The facility provides intensive, patient‑focused rehabilitation services, including physical, occupational, and speech therapy, to help patients regain mobility, strength, and independence after injuries or illnesses.
How you'll contributeAs a PPS Coordinator you will coordinate the timely and accurate completion of the Inpatient Rehabilitation Facility‑Patient Assessment Instrument (IRF‑PAI) for each rehabilitation inpatient, ensuring accurate scoring of functional status, reflection status, and care needs in the medical record and UDS Pro‑Doc system. You will work collaboratively with the interdisciplinary team to maintain data integrity, monitor outcome benchmarks, and support quality improvement activities.
- Conduct staff orientation and ongoing education to improve QI score accuracy at admission, throughout the stay, and upon discharge.
- Serve as an educator for QI scoring according to Uniform Data System (UDS) training materials.
- Provide a resource to licensed clinicians and clinical staff for interpretation of QI scoring guidelines.
- Ensure the IRF‑PAI is consistent with medical record documentation and reflects the 24‑hour care needs of patients.
- Collaborate with physicians and interdisciplinary team members to capture accurate patient assessment data in the UDS Pro‑Doc system and the medical record.
- Review medical record documentation and UDS Pro‑Doc data to support accurate functional scores and comorbid conditions, according to CMS guidelines.
- Compare actual patient knowledge to care provided and correct the IRF‑PAI as necessary to reflect the burden of care at admission, during stay, and at discharge.
- Investigate Impairment Group Codes (IGC) and co‑morbidities with physicians and coders to ensure accurate coding.
- Oversee and analyze QI data to ensure accurate completion of the IRF‑PAI before electronic transmission.
- Identify target reference dates for completion of the IRF‑PAI for all patients.
- Coordinate completion of the admission assessment by the fourth day of stay and the follow‑up assessment by the third day after discharge.
- Collaborate with the HIM department to ensure timely completion and transmittal of the IRF‑PAI to the CMS national database.
- Maintain current knowledge of UDS Pro‑Doc system and CMS IRF‑PAI software, providing expertise and contributing to facility process improvement plans.
- Keep updated on regulations concerning the IRF‑PAI completion within the inpatient rehab setting.
- Utilize UDS Pro‑Doc reports, UDS Pro‑Central reports, and related tools for data access, interpretation, and use.
- Serve as an information source and educator for IRF‑PAI and UDS users.
- Periodically perform medical record audits, including of the IRF‑PAI, clinical documentation, and billing information to ensure accuracy and consistency.
- Demonstrate effective critical thinking skills.
- Perform other duties as assigned.
- Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing, Nursing Diploma program, or licensed/registered PT, OT, or SLP.
- Current unrestricted state licensure as Registered Nurse (preferred) or PT, OT, or SLP.
- CRRN preferred.
- CPR/BCLS certification.
- UDSMR PPS Certification within three years.
- Three or more years of experience in the care of patients in an acute hospital or rehabilitation setting (preferred).
- Excellent oral, written, and interpersonal communication skills.
