Full-Time Hospice LVN

Mi Casa Hospice – San Antonio, TX

Mi Casa Hospice is seeking a compassionate, dependable, and motivated Licensed Vocational Nurse (LVN) to join our growing hospice team. This position provides direct patient care to hospice patients in their homes, assisted living facilities, and nursing facilities while working closely with the interdisciplinary team to ensure quality end‑of‑life care.

Responsibilities
  • Provide skilled nursing visits to hospice patients according to the plan of care.
  • Assess and document patient condition, symptoms, and changes in status.
  • Administer medications and treatments as ordered.
  • Educate patients, families, and caregivers regarding hospice care and symptom management.
  • Communicate patient needs and changes to the RN Case Manager and Interdisciplinary Team.
  • Complete all documentation accurately and within required time frames.
  • Participate in care coordination and patient care conferences.
  • Assist in maintaining compliance with hospice regulations and agency policies.
Requirements
  • Current Texas LVN License in good standing.
  • Minimum one (1) year of hospice experience required.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to work independently and manage a daily patient schedule.
  • Strong communication, organizational, and documentation skills.
  • Compassionate approach to end‑of‑life care and patient advocacy.
Position Expectations
  • Full‑time position.
  • Participation in an on‑call rotation is required.
  • Attendance at bi‑weekly Interdisciplinary Group (IDG) meetings is required.
  • Ability to provide compassionate, patient‑centered end‑of‑life care.
  • Maintain timely completion of all clinical documentation and visit notes.
