Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• CHPN certification
  - Preferred
• Proof of current CPR certification
• Current driver's license
• Reliable transportation and insurance
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
• Bachelor's of Science in Nursing
  - Preferred

Roles & Responsibilities:

• Assess and respond to patients' and families' needs in various settings.
• Develop, implement, document, and evaluate individualized Plans of Care (POC).
• Provide education to patients and families about care, medications, and home safety.
• Supervise and guide home health aides to ensure continuity of care.
• Ensure compliance with third-party payer requirements for assigned cases.
• Communicate effectively with healthcare team members and serve as a liaison for patients and families.
