Duties may include but are not limited to:
The incumbent will provide care for Veterans assigned to a Patient Aligned Care Team (PACT) as the clinical associate. Under the supervision of a Registered Nurse, the Licensed Vocational Nurse (LVN) applies the principles of advanced clinic access and Patient Aligned Care Team (PACT). The Licensed Vocational Nurse works closely with the interdisciplinary team and performs the following functions in a clinic setting:
- Completion of clinical reminders and health screenings, medication administration and immunizations;
- Pre-appointment calls, patient education, phlebotomy, diabetic foot exams, EKGs, checking vital signs, electronic documentation, secure messaging and other duties/tasks as assigned, including Primary Care Telehealth;
- Excellent customer service, interpersonal and communication skills are required, to include Primary Care Telehealth;
- Demonstrate knowledge, skill and ability to recognize and respond appropriately to emergency situations;
- Organizationally responsible to the Nurse Manager of the Clinic;
- In collaboration with registered nurses and/or physicians, provides a broad range of direct age-specific patient care activities and supportive services to selected acute care, long term, ambulatory care or psychiatric patients requiring complex, specialized care;
- Demonstrates knowledge of age associated changes, principles of growth & development and specific needs relevant to the adult and geriatric age groups;
- Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations;
- Support Women's Health, setting up women's health exams and treatments;
- Demonstrates knowledge and ability to appropriately carry out assigned patient care based on patients' conditions, including ability to observe, identify and respond to the patient's needs for medication, equipment-assisted care and patient/family education;
- Demonstrates knowledge and ability to assess, interpret data regarding patient condition; recognize urgent or emergent patient care situations;
- Demonstrates excellent clinical judgement; seeks assistance from the RN and/or MD/DO/NP to initiate appropriate emergency interventions as warranted by patient's condition;
- Observes, documents, and reports changes physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes and reporting any deviations from normal to RN or MD/DO/NP;
- Assesses risk for potential problems related to specific diagnosis and treatment;
- Assesses patients for evidence of elder abuse and follows correct reporting procedures;
- Assesses basic learning needs of patients and families with preventive health and/or discharge as a goal;
- Modifies communication/patient education to accommodate hearing, visual and cognitive impairments;
- Performs other duties as assigned.
Work Schedule:
Monday - Friday, 7:30am - 4:00pm, or 8:00am - 4:30pm;
Extended hours clinic rotation to include evenings & Saturdays (Tour of Duty and location may change due to patient care needs).
Pay:
Competitive salary and regular salary increases.
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave:
After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy:
After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
