Position: Lead Triage Hotline Nurse (RN)

Academic Req:
  • Required – Registered nurse license, Bachelor of Science in Nursing
  • Certifications:

    Good standing as a licensed Registered Nurse with the State of Texas Board of Nursing; current BLS certifications (PALS preferred), First aid, CPR, Emergency behavior intervention
  • Work experience:

    Required – At least 5 years of experience as a RN; and two (2) years of management experience
  • Preferred – pediatric nursing experience
Critical Action Items & Measurable Deliverables:
  • Meet and maintain all federal and state regulatory guidelines and standards applicable to this position.
  • Develop staffing schedules and assignments and provide solutions to resolve staffing gaps.
  • Provide telehealth triage services to assess patient needs and evaluate symptoms. This includes answering patient inquiries, providing patient teaching, and determining the urgency and appropriate course of action.
  • Conduct in-person and virtual assessments based on established protocols and accurately document medical findings.
  • Gather, collect, and clarify patients’ current condition and past medical history.
  • Effectively communicate with Medical Director as needed for reporting findings which are concerning for patient’s health and well-being.
  • Participate in the development of health and medical related protocols and procedures for federal programs.
  • Communicate with Lead Medical Coordinators and other medical staff to devise individual plans of care and acts as a nurse support to other medical staff.
  • Participate in scheduled meetings, team decisions and operations.
  • Implement Compass Connections safety protocols including evacuating with children and other staff in case of an emergency.
  • Maintain confidentiality of health care records and information in keeping with HIPAA and Compass Connections confidentiality and Personally Identifiable Information (PII) protocol and procedures.
  • Adhere to scheduled hours pertaining to position, remain available for on-call scheduling, and work evenings, weekends and holidays as needed, requested, or deemed appropriate by Regional Director and Executive Director.
  • Address other job duties as assigned or for unforeseen matters that require attention to maintain a fluid and fully functional program.
Other Responsibilities:
  • Review, write, and update medical protocols and procedures in collaboration with Medical Services and Compass Connections Professional Management Team.
  • Plan, organize and, prioritize daily work using sound judgement.
  • Monitor established health care plans for service population at the sites within the timeframe established by our State and Federal partners.
  • Ensure accurate documentation in accordance with agency policies and procedures, regulatory requirements and standards, and program timeline requirements.
  • Manage staff performance by providing immediate feedback, routine 1:1 coaching sessions and corrective action as necessary
  • Meet all program and federal partners’ documentation and processes requirements.
  • Other duties as assigned.
Requirements:
  • Pass a pre-employment drug screen and random drug screens throughout employment.
  • Provide proof of work eligibility status upon request.
  • Pass a pre-employment and biennial criminal background checks.
  • Demonstrate the ability to:
    • Work collaboratively with other staff members, service providers and professionals.
    • Communicate effectively in writing and verbally in English.
    • Work in a fast-paced environment and make decisions based on the needs of the program and service population, while maintaining emotional control and professional composure at all times.
    • Maintain computer literacy required to meet the responsibilities of the position.
    • Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
  • Demonstrate a working knowledge of all Compass Connections policies and procedures.
  • Must provide current physical-medical release and provide proof of immunizations, titers, or declinations for the following:
    • MMR
    • Varicella
    • Hep B
    • Hep A
    • Tetanus
    • PPD
    • Meningococcal (preferred)
