Treatment Nurse

Touchstone Communities is seeking a Treatment Nurse for our skilled nursing and rehabilitation community. In this role, you'll deliver direct clinical care, including treatments, wound care, and medication administration, while closely monitoring resident conditions and updating care plans. You'll collaborate with an interdisciplinary team to support recovery and long-term care goals in a homelike, faith-informed environment. This position is ideal for a compassionate nurse who values servant leadership, integrity, and person-centered care and wants to grow clinical and leadership skills.

Responsibilities

• Provide direct skilled nursing care to residents in a skilled nursing facility, following physician orders and care plans
• Administer medications, treatments, and wound care, accurately documenting all interventions
• Monitor and assess resident conditions, promptly reporting changes to providers and interdisciplinary team
• Collaborate with therapists, physicians, and CNAs to support rehabilitation and long-term care goals
• Educate residents and families on care plans, medications, and recovery expectations
• Ensure compliance with infection control, safety, and regulatory standards
• Participate in care conferences, documentation, and quality improvement initiatives
• Support and mentor CNAs and junior staff, promoting a culture of compassion and teamwork
