Job Summary:As an LVN with our team, you will provide skilled nursing care under the direction of an RN, assisting patients with daily activities, administering medications and treatments, and monitoring their physical and mental health. You will work closely with physicians and nursing staff on specialized procedures, document and report patient conditions, and support patients in achieving optimal well-being while upholding their rights and safety.
Key ResponsibilitiesProvide skilled nursing care in accordance with Texas Board of Nurse Examiners’ regulations and agency policies, including administering medications and treatments as directed.
Assist patients with activities of daily living, promote self-care, and observe, assess, and document their physical and mental health accurately and timely.
Ensure compliance with health programs, including Medicare Conditions of Participation and Coverage for skilled services, and properly use durable medical equipment.
Use point-of-care software and clinical pathway tools efficiently to support patient care.
Apply strong clinical judgment, attention to detail, and maintain excellent communication and organizational skills.
Collaborate with the interdisciplinary team—including RNs, therapists, physicians, and social workers—attending team meetings and case conferences to ensure coordinated care delivery.
QualificationsValid Texas driver’s license with reliable transportation and ability to travel safely to patients’ homes.
Physical ability and strength to assist patients with mobility, perform assessments, treatments, and procedures (e.g., IVs, blood draws), and handle medical supplies and equipment.
Adequate vision, hearing, touch, and manual dexterity to safely and accurately use electronic devices, documentation software, and perform clinical tasks.
Strong organizational, time management, communication, and multitasking skills to prioritize care and meet deadlines in varied environments.
Education, Training, and Professional Credentials:Completion of a practical (vocational) nursing program and current licensure as a Licensed Vocational Nurse (LVN) in Texas or a multi-state NLC license.
Provides care under the supervision of a registered nurse (RN).
Minimum of 1 year of direct patient care experience in a clinical setting (home health, hospital, skilled nursing, or long-term care).
Knowledge of clinical practices and ability to apply training in patient care effectively.
