United Health Group's CHRISTUS VNA Hospice, part of the Optum family, is seeking an RN Case Manager to lead the Plan of Care for hospice patients and collaborate with the IDG and attending physicians. You will evaluate care needs, coordinate treatment strategies, and support families with compassionate nursing care.
Current RN licensure in applicable state, driver’s license, CPR certification, and 1+ year of clinical experience are preferred.
