Role:
Licensed Vocational Nurse (LVN) providing skilled nursing care in patients' homes, primarily for pediatric and adult patients. Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services in accordance with prescribed orders.
- Monitor signs and symptoms to identify physiological and psychosocial changes.
- Collect, analyze, and interpret data from healthcare team members, documenting nursing diagnoses and patient status.
- Current LVN license in the practicing state.
- Current TB test or Chest X-Ray and BLS certification.
- Preferred: at least one year of professional nursing experience.
- G-tube, tracheostomy, and ventilator experience preferred, with select training offered.
Provide patient-centered nursing care, monitor health status, and ensure adherence to prescribed treatment plans.
