Gentiva Hospice is seeking an After hours RN Care Manager to deliver high-quality hospice care during nights, weekends, and on-call hours. You will provide direct patient care, perform assessments, and update plans of care with physicians, ensuring continuity through the IDG.
The role requires at least 1 year of hospice experience, an active Texas RN license, strong documentation, and leadership in a fast-moving after-hours team. Schedule is 7 days on, 7 days off.
