CHRISTUS VNA Hospice, part of the United Health Group family, is hiring a Registered Nurse Case Manager to deliver and manage the Plan of Care for hospice patients in the community. You will evaluate needs, coordinate care with the IDG, and implement treatment strategies to support physical, psychosocial, and spiritual well-being.
Qualifications include an active RN license in the state, a valid driver's license, and CPR certification. Prior hospice or home health experience is preferred.
