Patient Outreach Coordinator - LVN​/LPN

Position Overview

We are seeking a licensed LVN/LPN to support a healthcare quality team focused on HEDIS and STARs measures, patient outreach, and closing gaps in care.

This position combines clinical knowledge, patient education, data review, and a significant amount of telephonic outreach. The LVN/LPN will work with patients who have outstanding quality measures or gaps in their healthcare and help determine what is needed to close those gaps.

A large portion of the day will involve outbound calls to patients. During these conversations, you may educate patients on recommended screenings or preventive care, discuss why a particular measure is important, help coordinate appointments, and address barriers that may be preventing the patient from completing their care.

This is a great opportunity for an LVN/LPN who enjoys patient interaction but is interested in a role that is less focused on traditional bedside nursing and more focused on care coordination, population health, quality improvement, and patient education.

Key Responsibilities

+ Conduct a high volume of outbound calls to patients regarding open healthcare quality measures and gaps in care

+ Educate and counsel patients on HEDIS, STARs, preventive care, and other quality measures

+ Identify patients who are missing required or recommended healthcare services

+ Help patients understand what steps are needed to close their care gaps

+ Assist with appointment scheduling and coordination when appropriate

+ Review patient information and healthcare data to identify missing or incomplete measures

+ Evaluate the quality and completeness of clinical gap closures

+ Accurately document patient conversations, outreach attempts, outcomes, and follow-up needs

+ Analyze information for potential data discrepancies or missing documentation

+ Partner with internal healthcare teams to resolve issues or barriers impacting quality measures

+ Follow established clinical, documentation, privacy, and regulatory guidelines

+ Maintain current knowledge of HEDIS and STARs standards and requirements

+ Meet established productivity, quality, and outreach expectations

Requirements

What They Are Looking For

The most important requirement for this position is an active LVN/LPN license. Candidates should also be comfortable spending a significant portion of their day on the phone communicating directly with patients.

The strongest candidates will have previous experience in areas such as:

+ HEDIS or STARs

+ Quality improvement or quality measures

+ Care coordination

+ Population health

+ Managed care or health plans

+ Patient outreach

+ Preventive care

+ Telephonic nursing

+ Healthcare call center environments

+ Medical record or clinical documentation review

Direct HEDIS/STARs experience is highly preferred, but candidates with a strong LVN/LPN background and relevant patient outreach, quality, or care coordination experience may also be considered.
