As a Hospice RN Weekend On Call Case Manager, this position reports directly to the Patient Care Manager and is responsible for providing skilled professional nursing care to the patient and family. The incumbent oversees quality of care delivery by all disciplines and programs, assures delivery of optimal care safely and effectively, and identifies both existing and potential problems that impact the quality of patient care.

Also ensures that provision of the highest quality of care to residents receiving Hospice services. This role is critical in directing, assessing, planning, implementing, and overseeing the care provided to patients. The incumbent occupies a position of confidence and trust and must maintain confidentiality in accordance with Health Insurance Portability and Accountability Act regulations.

• Provides patient care in accordance with accepted standards of nursing practice, agency policies and procedures, and the written plan of care to include initial and ongoing assessments of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial, spiritual and environmental needs, and on activities of daily living.
• Receives patient referrals and applies specific criteria for admission and re-certification for hospice care to establish appropriate levels of care and the patient’s eligibility.
• Coordinates, assesses, implements, and revises the individualized plan of care, to include:
  Managing discomfort and providing symptom relief, providing specialized nursing skills related to palliative and end-of-life care, creating plans of care and clinical solutions to patient problems utilizing the nursing process and critical thinking skills, and determining scope and frequency of services needed based on acuity and patient/family needs.
• Consults with and educates the patient/family regarding the disease process, self-care techniques, end-of-life care, and the process for dealing with issues of ethical concern.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Participates in the coordination of all patient family services and prioritization of needs with the members of the interdisciplinary team.
• Provides field supervision to Licensed Vocational Nurses, Home Health Aides, and personal attendants assigned to the incumbent’s case load according to regulations and guidelines.
• Attends and participates in ongoing quality assurance and performance improvement activities as assigned.
