Licensed Practical / Vocational Nurse – LPN/LVN

Overnight shifts (Friday – Sunday, 7 a.m. – 7 p.m.).

The Licensed Practical Nurse (LPN/LVN) will provide and document skilled nursing care under the supervision of a Registered Nurse in accordance with the care plan and physicians' orders for each individual patient, while maintaining confidentiality and professional boundaries.

• Deliver and coordinate quality patient care in compliance with physician orders.
• Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
• Document all activities, assessments, nursing actions, responses, and coordination of care promptly.
• Participate in, implement, and update the nursing care plan.
• Take appropriate nursing action based on assessment to achieve expected outcomes.
• Identify changes in patient needs and responses requiring intervention, and implement care to prevent or reduce risk.
• Adhere to Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
• Apply infection control measures that protect staff and patient according to OSHA standards.
• Educate patient and family on disease processes, self‑care techniques, and prevention strategies.
• Maintain knowledge of nursing competencies through continuing education and skill evaluations.

• Active, unencumbered LPN/LVN license in the state of practice; compact licenses must be transferred to state of residence within 90 days.
• Certification in CPR with a hands‑on component (Aveanna may assist with obtaining after hire if required).
• TB skin test current within the last 12 months.
• At least six months of hands‑on nursing experience preferred but not required.
• Reliable transportation.

• South Carolina – one year of pediatrics experience.
• California – one year of experience under current nursing license.
• Louisiana – one year of experience as a licensed nurse.
• Continuing education as required by state.

• Six months of recent experience as a licensed nurse in a clinical care setting.
• Home health experience.

• Attention to detail.
• Time management.
• Effective problem‑solving and conflict resolution.
• Strong organization and communication skills.

• Speak, read, write, and understand English.
• Travel ability.
• Lift up to 50 pounds and reposition patients and equipment without assistance.
• Prolonged walking, standing, bending, kneeling, reaching, twisting, sitting, and climbing stairs.
• Adequate visual and hearing acuity; strong sense of smell and touch.
• Respond physically and mentally to emergency situations in the home or during transport.

• Work in diverse environments that may involve exposure to allergens and various conditions.
• Possible exposure to blood, bodily fluids, and infectious diseases.
