Lead Case Manager/Licensed Chemical Dependency Counselor (LCDC) provides substance‑abuse treatment through individual and group counseling services to clients with substance use disorders and co‑occurring behavioral health challenges. The role supports case managers while maintaining a personal caseload.
Essential Functions- Support the Program Director in managing the referral process and ensuring efficient case assignment, including:
- Reviewing and triaging new referrals by monitoring caseload capacity and case complexity.
- Facilitating timely intake and engagement with youth and families.
- Monitoring referral patterns and collaborating with supervisors to identify service delivery gaps.
- Case Support & Guidance:
- Act as a resource for case managers, providing guidance on complex cases and intervention strategies.
- Support case managers in engaging families, troubleshooting challenges, and connecting them with appropriate services.
- Provide feedback on documentation, case notes, and reporting requirements for compliance.
- Collaboration & Program Development:
- Identify service gaps and relay concerns or recommendations to program leadership.
- Participate in team meetings to ensure consistency in service delivery.
- Assist in training new case managers and provide ongoing peer support.
- Monitor compliance with program requirements, agency policies, state regulations, and accreditation standards.
- Participate in required trainings, supervision, and professional development activities.
- Attend at least three community events per year on weekends to represent the program and engage with the community.
- Chemical Dependency Counseling:
- Conduct substance‑abuse assessments, screenings, and evaluations.
- Develop, implement, and update individualized treatment plans.
- Provide individual, group, and family counseling using evidence‑based practices.
- Facilitate psycho educational and therapeutic groups focused on recovery, relapse prevention, life skills, and behavioral health topics.
- Monitor client progress and adjust interventions as appropriate.
- Maintain timely, accurate, and complete clinical documentation.
- Provide direct supervision and support to youth and families in the home, school, and community.
- Conduct intake assessments to identify root causes of truancy and develop individualized case plans.
- Work closely with families to address barriers to attendance such as mental health, substance abuse, transportation, or instability.
- Assist families in accessing community resources.
- Maintain confidentiality of client information in accordance with HIPAA.
- Crisis Intervention & Advocacy:
- Participate in after‑hours support rotation, providing immediate response to urgent needs.
- Assist families in addressing crises that could lead to legal consequences, school dropout, or out‑of‑home placement.
- Support case managers in high‑risk cases with additional guidance.
- Advocate for families by coordinating services and interventions that support school attendance and success.
- Collaboration & Community Outreach:
- Develop professional relationships within schools and the broader community.
- Participate in staffings, training sessions, and educational workshops to enhance family awareness of truancy laws.
