The Texas Health and Human Services Commission (HHSC) is seeking a Social Worker III to support individuals with traumatic brain injury (TBI) and/or spinal cord injury (SCI). The position requires extensive case management, development of person‑centered rehabilitation plans, and coordination of services to achieve rehabilitation goals. Telework is eligible, and travel up to 40% may be required.

Essential Job Functions
  • Collect and analyze information necessary for accurate eligibility decisions, including medical, psychological, and school records. Conduct comprehensive assessments using trauma‑informed care and person‑centered practices. (10%)
  • Develop and implement person‑centered rehabilitation plans that identify short‑term and long‑term goals, objectives, and required actions. (10%)
  • Coordinate the purchase of goods, services, and supports using the wraparound model, ensuring timely, cost‑effective provision of services and supports that advance clients’ goals. (25%)
  • Provide case management, resource facilitation, and discharge planning to maintain continuity of care and ensure all needs are addressed before case closure. (20%)
  • Offer education, guidance, self‑advocacy support, and technical assistance to persons with TBI/SCI and their families throughout program participation. (10%)
  • Utilize written and verbal communication with internal and external stakeholders to maintain positive relationships and document all case‑related actions in the electronic case management system. (10%)
  • Respond to inquiries from HHSC staff, service providers, other state agencies, and the public; provide technical assistance and support policy implementation. Participate in program policy and procedure development, contract monitoring, evaluation, and budget planning. (5%)
  • Serve as a subject matter expert on TBI and SCI for stakeholders, clients, and families; maintain a knowledge of community resources and make referrals as needed to ensure appropriate use of state funds. (5%)
  • Perform additional duties as assigned to support the Comprehensive Rehabilitation Services program mission and goals. (5%)

Initial Criteria
  • Graduation from an accredited four‑year college or university with major coursework in social work, counseling, or a related health and human services field.
  • Two years of experience providing case management, assessing needs, and developing treatment plans.
  • Experience working with individuals with TBI, SCI, or other disabilities.
Preferred Criteria
  • Master’s degree in social work, counseling, or a related health and human services field.
