Job TitleSoc Svcs Surveyor (Hlth Fac)
Position SummaryAs a Nursing Facilities Social Services Surveyor, you will conduct on‑site inspections and investigations of Texas long‑term care facilities (nursing homes, assisted living, day‑activities, and health services) to ensure compliance with state and federal laws and regulations. You will write detailed reports on deficiencies, propose remediation plans, and support regulatory determinations.
Essential Job Functions (EJFs)- Conduct licensure inspections, complaint and incident investigations, and annual comprehensive surveys under the direction of a program manager.
- Complete inspection or survey reports detailing deficiencies and corrective actions.
- Communicate effectively with internal and external stakeholders to provide, exchange, or verify information regarding social services in long‑term care facilities.
- Conduct exit conferences with facility staff regarding findings.
- Verify correction of previously cited regulatory violations through follow‑up visits.
- Perform quality assurance reviews of documents prepared by others.
- Occasionally testify at hearings or in court proceedings to provide information regarding policy, procedures, history, findings, or activities.
- Complete referrals as appropriate.
- Provide limited on‑site assistance in emergency licensure action or for skilled care if a registered or licensed nurse.
- Serve on‑call for emergencies such as weather events or high‑priority situations.
- Interpret policies, procedures, and regulations.
- Work independently.
- Use a computer for report writing.
- Exercise professional judgment and reach sound decisions.
- Integrate complex regulatory concepts, procedures, or processes.
- Multi‑task and track multiple activities with competing priorities.
- Must hold Surveyor Minimum Qualifications Test (SMQT) Certification or obtain it within 12 months of hire.
- Licensed to practice as a social worker by the Texas State Board of Social Work Examiners.
- Experience in assessment and documentation of social service needs.
- Experience working as a social worker in an acute or long‑term health care setting (preferred).
- Flexibility in work hours is required.
- Position requires 80% travel, including statewide, overnight, and weekend travel.
- Certain on‑call duties for emergencies or high‑priority situations may be required.
- Applicants must pass a fingerprint criminal background check.
Salary: $6,155.63 – $6,779.25 per month (Monthly) – Starting Market Rate Salary $73,867.50 annually.
