Summary
The Case Manager is responsible for providing comprehensive case management and coordinated supportive services to eligible veterans, surviving spouses, and dependent family members seeking assistance through the Veteran and Family Relief General Assistance Program funded by the Texas Veterans Commission. The Case Manager conducts client assessments, determines program eligibility, develops individualized service plans, and provides emergency financial assistance for eligible rental, utility, and other approved basic needs in accordance with grant requirements.
DescriptionThe Case Manager is responsible for providing comprehensive case management and coordinated supportive services to eligible veterans, surviving spouses, and dependent family members seeking assistance through the Veteran and Family Relief General Assistance Program funded by the Texas Veterans Commission. The Case Manager conducts client assessments, determines program eligibility, develops individualized service plans, and provides emergency financial assistance for eligible rental, utility, and other approved basic needs in accordance with grant requirements.
The Case Manager works collaboratively with participants to promote eviction prevention, housing stability and long-term self-sufficiency by connecting them with employment services, workforce development, financial capability education, benefits enrollment, and other community resources.
Additional responsibilities include client outreach and engagement, comprehensive case management, documentation of eligibility and services, maintenance of grant-compliant client files, data entry, performance and outcome tracking, monthly reporting, and ensuring compliance with all Texas Veterans Commission, agency, and funding requirements. The Case Manager is expected to maintain strong partnerships with veteran-serving organizations and community providers while delivering services with professionalism, compassion, and a commitment to helping veterans and their families achieve lasting stability.
RequirementsPosition Responsibilities
- Conduct comprehensive intake, eligibility screening, and needs assessments for eligible veterans, surviving spouses, and dependent family members in accordance with Texas Veterans Commission (TVC) Veteran and Family Relief grant requirements.
- Verify client eligibility by reviewing required documentation, including DD Form 214, proof of military service, income, residency, household composition, and other grant-required documentation.
- Develop individualized service plans that address immediate financial needs and establish goals toward housing stability, financial security, and long-term self-sufficiency.
- Provide comprehensive case management, including ongoing client engagement, progress monitoring, reassessments, and follow-up services.
- Determine eligibility for emergency financial assistance in accordance with grant guidelines and agency policies.
- Provide referrals to internal Catholic Charities programs and external community partners to address identified needs.
- Maintain an active caseload while ensuring timely documentation, follow-up, and continuity of services.
- Complete all case notes, assessments, service plans, eligibility documentation, and supporting records accurately and within required time frames.
- Enter client information and program data into all required agency and grant databases in a timely and accurate manner.
- Maintain complete, organized, and grant-compliant client files in accordance with agency policies and Texas Veterans Commission requirements.
- Monitor participant progress toward identified goals using approved assessment tools and document measurable client outcomes.
- Conduct file reviews and quality assurance activities to ensure compliance with grant requirements, agency policies, and applicable federal and state regulations.
- Develop and maintain collaborative relationships with landlords, property managers, veteran-serving organizations, employers, educational institutions, workforce agencies, and other community partners to increase access to resources for program participants.
- Participate in outreach activities and community events to increase awareness of Veteran and Family Relief services and recruit eligible participants.
- Represent the organization professionally in meetings, case conferences, networking events, and community collaborations as directed by Associate Senior Director or Senior Director.
- Travel as necessary to conduct client visits, attend meetings, coordinate services, deliver payments or documentation to vendors, and support program operations.
- Must complete all required professional development assigned in a timely manner.
- Must be sensitive to the service population’s cultural and socioeconomic characteristics.
